Privacy Request

Use this page to request access to, correction of, or deletion of personal information that Trinity Wellness, LLC d/b/a Ordeva holds about you in connection with ordeva.org and related marketing activities. Privacy requests are handled at privacy@ordeva.org.

How to Submit a Request

Email privacy@ordeva.org with:

• Your full name and the email address associated with your inquiry or account
• The type of request (access, correction, deletion, or other)
• Enough detail for us to locate your information

We may need to verify your identity before fulfilling a request. We will respond as soon as reasonably practicable, consistent with applicable law.

Do Not Include PHI

Do not include patient names, medical records, laboratory results, diagnoses, medications, or other protected health information in a privacy request sent through this website channel. This channel is for website and marketing personal-information requests only.

Requests related to PHI processed for a covered clinic under a Business Associate Agreement should be directed through the clinic's designated process or the contacts specified in the applicable customer agreement and BAA.
